The Interprofessional Care for People with Multiple Chronic Conditions (IPCMCC) modules will prepare you to work in interprofessional teams to foster self-management by patients with multiple chronic conditions through the application of proven coaching strategies. The ICPMCC materials focus on working within an interprofessional collaborative practice in outpatient settings, with an emphasis on coaching to enhance patient engagement and improve patient self-management skills. The materials integrate the Interprofessional Education Collaborative (IPEC) interprofessional education competencies (IPEC Expert Panel, 2011) with the Chronic Care Model (Wagner, et al, 2001). All learning materials stress the importance of patient and family-centered care and the role of the community in supporting health and health care.

 

To get started:

  1. Create a new account and login.
  2. Choose Course Catalog.
  3. Click the Interprofessional Care of People with Multiple Chronic Conditions link.
  4. Click Enroll.
  5. Click My Account.
  6. Click View Course button in the View column on the far right.
  7. Click the Launch Lesson button in the Action column on the far right.

 

When you have completed all of the modules, click the My Activities link. You will see a link beneath the calendar to print or save your completion certificate as a .pdf.

This comprehensive learning package includes a suite of complementary activities that can be used individually or together to enhance the health professional curriculum, including:

 

  1. Descriptions of each module, reflective discussion questions and key teaching points to complement this self-paced eLearning course;
  2. A collection of online collaborative learning activities through which learners can apply new learning through peer dialogue in the context of case-based scenarios; and
  3. Reflective practice questions to guide the integration of learning from clinical experiences.
